Important Policy Information for Current Clients
Medication Refill Policy
Please call 5-7 business days prior to your medication running out as this will ensure a timely refill. If your request is submitted outside of business hours, please advise the refill will be submitted to your preferred pharmacy up to 72 hours. We do not accept medication refills directly from your pharmacy.
Controlled Substance Policy
Our providers strive for medication safety in each treatment plan created. Our providers strive to minimize the use of controlled substances; therefore, we create a safe treatment plan to reduce and eliminate the use of controlled substances.
PLEASE NOTE - controlled substances may NOT be refilled if an appointment is missed or rescheduled beyond the original recommended follow up.
Missed Appointment/No Show
Please be advised a cancellation fee of $75 will be imposed if an appointment is missed or cancelled within 48 hours.

Emotional Support Animal Policy
We understand your furry friends are part of your support system and promote your mental health; however, we are unable to provide letters or consent for such service at this time.
You may visit www.supportpets.com for an online ESA approval.

Our Recommendations
We work closely with Quest Diagnostics if this does not work for you however most primary care providers can also run labs. If you have any questions or concerns please reach out to our office.
